OHIO Screening Tool

Many individuals with a brain injury may not identify as someone with a head injury, nor may they have a formal diagnosis on their file.  Sometimes a simple question like “Have you been hit on the head?” as opposed to “do you have a brain injury?” may elicit a discussion revealing the type of information you are looking for.  This Screening Tool by the Ohio Valley Center for Brain Injury Prevention and Rehabilitation can be helpful in outlining questions you can ask your client to determine if they have a history of brain injury and some information on how you can assist and refer them for support.   If you suspect they may have a brain injury and are not sure how to proceed with support, contact The Brain Injury Society of Toronto, or the Ontario Brain Injury Association (*add link to contact information).

This information should be used to help identify if your client has a suspected brain injury, it should not be used as a diagnostic tool.  Use this TBI identification tool to:

  • Help identify and address your client’s symptoms/challenges
  • Understand how it might affect them during their court process,
  • Determine what medical records can be retrieved
  • Decide if they need to be referred to a brain injury association and connected with ABI supports/resources.
